What does an AI scribe leave out of the note?

Mostly the things said quickly, in passing, or buried in a long story, and sometimes the thing that mattered most. In a randomized trial of two AI scribes, omissions were reported more than any other error type, twelve times, just ahead of formatting problems at eleven. A psychiatrist said their tool missed large parts of the symptom history. A healthcare governance lead put it plainly: a note can be accurate and still incomplete.

What gets left out?

Details that were mentioned once and never repeated. Symptom history told out of order. The diagnostically important sentence in the middle of a long answer. A psychiatrist using an institutional tool said it was poor at recognizing exactly those parts. A nurse practitioner who tried several tools said one of them left out much of what they had covered in the appointment.

Why is an omission worse than an error?

Because there is nothing on the page to check. A wrong dose is visible if you read carefully. A missing one is not. The governance lead's point was that a note can be well written and factually accurate and still leave out something that changes the next treatment decision. The clinician may not notice. A reviewer may not notice. A later system reading the chart cannot retrieve what was never written down.

How do I catch what is missing?

You cannot proofread for an absence, so check the draft against a short list you carry in your head: every medication changed, every dose, the one thing you told the patient to watch for, and when you want to see them again. If any of those is not on the page, add it.

Can I reduce omissions?

Say the important things clearly and in full, especially medications and doses. Summarize the plan out loud at the end of the visit. And decide who should write the plan, because the part you write yourself is the part least likely to be missing.
