The second is clinical suggestion: you did not ask about this symptom, the guideline says consider that test. Both are marketed as the tool catching what you missed. They carry very different weight.

What is the risk?

A suggestion that looks authoritative gets accepted without thought. A tool that suggests can also miss, and omissions are among the errors clinicians report most. And whatever ends up in the note, the responsibility sits with the clinician who signs it.

Where is the line?

A reminder about what the note lacks is a checklist. It helps, and you can ignore it safely. A clinical suggestion is advice. It may be right, but you have to weigh it as you would advice from any source, and it should never be written into the note as if you had decided it.

What should I look for?
