Does an AI scribe know the difference between what the patient said and what is true?

No. A podiatrist who runs a two-person practice listed it first among the tool's quirks: it faithfully records as true whatever the patient claims. A psychiatrist found the reverse problem, with their own screening finding written up as the patient's complaint. A large trial counted confusion between yes and no as an error type of its own. The tool records words. Deciding who is vouching for them is still your job.

What goes wrong?

Three things. A patient's claim becomes a fact: the note says the patient has something, when the patient only said they did. The clinician's finding becomes a patient report: the psychiatrist screened a patient positive for a sleep problem, and the note made it sound as if the patient had come in about snoring. And negatives flip: denies chest pain becomes chest pain.

The trial of two AI scribes across 238 physicians recorded five reports of yes-and-no confusion, alongside omissions, formatting problems and pronoun errors.

Why does it matter?

Because the next person to read the note will treat it as fact. A claim recorded as a finding can follow a patient for years. A finding recorded as a complaint changes why the visit looks like it happened, which matters for billing and for anyone reviewing your reasoning later.

How can I catch it?

Read for attribution, not just accuracy. Look for the difference between reports, states and denies on one side, and plain assertions on the other. Check every negative. And ask the tool to keep the patient's account and your findings in separate sections, if it allows it.
