Do I have to narrate my exam for an AI scribe to catch it?

Mostly, yes. A podiatrist explained that the tool captures exam findings only when they are said aloud, and a colleague asked whether that meant narrating every exam in front of patients. Clinicians in another forum said their tools were weak on the physical exam unless they did. The exam is seen, not heard, and the tool only hears.

Why can't it catch the exam on its own?

Because it works from audio. Palpation, inspection, a range of motion, a gait: none of it makes a sound. Unless you say what you found, there is nothing for the tool to write. One clinician with a fast hospital tool simply does not use it for the exam at all.

What does narrating cost?

Some awkwardness. Patients hear your findings as you make them, which can reassure one person and alarm another. It adds a little time. And it changes the feel of the visit, which is worth thinking about before you make it a habit.

How are clinicians handling it?

Three ways. Say the findings briefly as you go, in plain words the patient can hear comfortably. Dictate the exam after the patient leaves, when the tool is listening to you alone. Or keep the exam in your own template and type it as you always have.

What should I never do?
