Is it worth building your own AI scribe?

For a few clinicians, yes, and what they built is telling. A cash-pay practice owner in a psychiatry forum put together a local setup for under a thousand dollars because they would not send patient audio to the cloud. A pain physician was working with developers on their own after finding commercial tools either too rigid or too buggy. A physician wrote an app to move notes into their records system with hotkeys. Why are clinicians building their own?

Three reasons, one per builder. Privacy: that owner's patients pay partly for discretion, and a cloud service broke that. Control: the pain physician found one product natural but hard to customize and another customizable but unreliable. Integration: the third physician's records system had no good connection, so they made one.

What does it take?

More than the hardware. You need the skill to set it up, the time to maintain it, and the willingness to be your own vendor. That includes security: if you build it, protecting the recordings and notes is entirely your job, and so is fixing it when it breaks between patients.

What is the risk?

Your time, first. Then reliability, with no support line. Then security, with no one else to share the responsibility. Most clinicians who try this are technically confident. Most clinicians are not, and should not have to be.

What do these builds tell buyers?

Where the market falls short. Keeping audio on your own device. Writing in your own format. Putting the note where you sign it. If three different physicians had to build those three things, they are worth asking any vendor about directly.
